When writing a book and formatting and whatnot, is it necessary to create a Table of Contents for all the chapters or is that what a publisher does? Sorry if all my topics are all questions and stuff, but I'm so new to all this (well, to the publishing side of this) that it's all rather...daunting.

I've never heard of that being a requirement. Your pages on your document aren't going to be the same page numbers as a final published product anyway.
