The animal protected her and died after the 10th tribute went down, she actually died when the careers turned on one another and almost won but was outsmarted by a District 6 tribute that was hidden until the end and caught her by surprise.
Edit: Also about the name poll, why can't it be Katniss + Peeta = Kateeta? Or Katniss + Gale = Kale?

Stefani, a tribute from 4, was picked for the same games as Chocolate, a returning career from 2.

Stephani thought the dress was an obvious hindrance to the game, but discovered that Chocolate held a good stash of weapons underneath. She'd learned a fair amount in the previous game, and took the former out with a well hidden piano wire not long after she'd discovered Stefani's whereabouts.
Stefani would be the 8th to fall, but Chocolate would get taken out by a crafty tribute from 11, who'd used a nearby water source and Chocolate was taken out via drowning when he shoved her in just before sunset using the background light to his advantage. She'd been one of the last 6.
