Some fetishes may come from classical conditioning. My psychology teacher told us about this guy who had a sock fetish. It was so bad that he would prefer masturbating with a sock instead of having sex with his wife. His fetish started when he was a teenager and would always masturbate with a sock, which means he unknowingly conditioned himself to get off with just a sock.

In case you're wondering, they fixed the problem by incorporating the socks with sex, like having his wife wear socks during sex. Simple enough solution. I don't know why they wouldn't have tried it before paying money for a therapist. :/
