I believe it's 25px high by the width of the panel.

For the panels in the first column, the dimensions are 210px wide and 22px high. The panels in the second column are 480px wide.

There are a few different ways to add a header to v2 profiles. The one I recommend is setting the header image as the body background, and setting the page background as the html background. Doing this allows you to add a footer as well.

Here's a quick example I made: http://www.gaiaonline.com/profiles/5498755

This is my code for the header and body backgrounds:
body {background: url(http://i109.photobucket.com/albums/n79/kayleyicons/bleachheader7.png) top center no-repeat; height: 371px; width: 1000px;}
html {background: #EFE6E1 url(http://i109.photobucket.com/albums/n79/kayleyicons/sidebar3.png) center right no-repeat fixed;}

I also had to add a relative top positioning of 351px to #columns to push them below the header.

Gosh, making that example makes me want to create a v2 premade. rofl

I'll add something to the specific codes section about headers and footers later.
