how about one for phaeton like a burning chariot? because he tried to ride helios' sun chariot and he fell to the earth and then zeus shot him with the thunderbolt so the earth wouldnt burn...

or tithonus...even if its not much of a curse...he was eos' husband and eos asked zeus to give tithonus immortality but she forgot to ask for tithonus to have eternal youth so he grew SO OLD he became a cricket (or grasshopper)...the avatar could be all wrinkly or be a cricket...

OR you could have circe's curse where the avatar has pig body parts or is a pig...
