there are four different order forms. these all have different purposes, so please be sure that you are using the right one.
ORDERFORM 1 - use this to order a fish for yourself
ORDERFORM 2 - buying fish for other people? use this one
ORDERFORM 3 - use this if you already have FISHBOWL fish and would like to order accessories and/or additional fish
ORDERFORM 4 - this is for giftcards only.
