1) Grip the bottle/jar lid with your palm and push down while you twist.



That tends to work in most situations for me. The trick is to use your forearm while twisting, a large muscle group, rather than those in the fingers. Pushing down forces the forearm to lend its strength to the motion
Here is a trick I learned in cooking school... Take a cloth on the counter and fold it up. Take your jar turn it upside down and bang it on the counter. It loosens the lid enough so you can unscrew it.
I don't usually have a hard time but when I do, I just put the jar under warm water for a bit and open it with a cloth.
