Hi all, i want to build a Slim Form Factor (SFF) PC, so wondering if anyone knows what graphic card gives the best performance in graphocs and yet runs really cool and stable inside the tiny SFF casings?

Is this for some sort of challenge, does it 100% NEED to be slim?
'Cause going non-slim could net you double the graphical power.

And what will this system be used for? If you're looking for high-end gaming performance you simply won't find it in a low profile card. If you're just looking for hardware accelerated video playback then any entry-level card (preferably passively cooled) would be fine. If you're looking for something in between, you should be able to find something but the internal temperature won't be very cool. Not something to worry about though, as computer components have very high heat thresholds.

Short answer: No.
Long answer: I don't like making fun of people with mental handicap, so I won't. You may want to look into gaining disability payments and hiring a helper to make these kind of decisions for you.
