I have a feeling that Katara never taught anyone how to blood bend. It's kinda the dark side of bending, if you ask me, and she's a goodie two shoes.
That doesn't account for anyone else figuring it out on their own, of course.

I don't even know how there are only lightning and metal benders, lightning comes from fire bending and metal comes from Earth. Metal and lightning are not part of the 4 elements.
