Well, they give you this wax stuff that you can put on the braces if they start rubbing, but you can't put it on sections that are just the wire; it won't stay. Unfortunately, it's the wire that can be sharp. ):

Not very often. I've had my braces for about a year, and it's only happened once or twice.
And yes, they should. But they don't.
