For my favorite anime I have a tie between Blood+ and Chrome Shelled Regios! i love action animes and these are filled with a lot of action.

and both is Death Note (i have read the whole manga a the first about 10 episodes of the Anime... and they are both good ^^)

Best anime is a hard question for somebody to answer. I think one of the best animes out there is Hellsing OVA. But I also love Lament of the Lamb, Bleach and Ouran high school.

Best manga almost has to be Angel Sanctuary or Hellsing, I've read both to the end and they are fantastic and full of dark humour.
