Okay to cover your first few question: Yes, Fate/stay Night started as an adult visual novel. Think a choose your own adventure book, with pretty backgrounds and character portraits displaying who's talking dialogue. You make choices, and get a number of endings based on them (many of them being "bad" endings). It happened to feature detailed sex scenes as well as associated imagery, giving it an adult rating. Because you don't sell porn to kids.
Mind you, the whole thing isn't porn or anything. The story's actually quite nice, and quite lengthy at that. I'd say the sexual content makes up maybe 15% of the whole thing. Maybe a little more, given Heavens' Feel.
Most of it's pretty easy to skip over though.
I'd thoroughly and wholeheartedly recommend reading through the visual novel first. The DEEN adaptation was literally garbage, both the anime and the UBW movie (the latter being nothing more than a shiny, two hour long music video).
There are two issues: One, if you don't want to spend a lot of money on an actual copy of it, you'll have to dabble in doing things a way that I can't tell you much about. Pirate it. That's what I'm saying.
Two, you'll have to patch it with Mirror Moon's english patch. that part's not against the rules or anything.
Play the visual novel through.

I wouldn't say 15%. For a 48 hour long game, I spent less than 30 minutes on the sex scenes. I'm just counting the four main sex scenes, not the other content of a sexual nature.
I was going with most of the stuff of a sexual nature really. The major scenes are very much limited though, yeah.
